Classify internal audit findings
Objectives
Northstar's first internal audit produced six findings. Accept the ones that are only observations (no requirement is unmet) and reject the rest as a major or minor nonconformity.
Room rubric: major means a required process is missing or has failed across the board. Minor means an isolated failure in a process that otherwise works. Observation means an improvement idea with no unmet requirement.
Verdicts are accept or reject. Every rejection needs one reason code from: major | minor.
Six internal audit findings
A-01 | No internal audit programme exists and no audits were held in the last twelve months. A-02 | One of twelve monthly access reviews was done eleven days late; the other eleven were on time and complete. A-03 | The auditor suggests clearer colours on the risk dashboard. Every required risk report was produced and reviewed. A-04 | Management review has never been held, so no decisions on resources, risks or objectives were recorded. A-05 | The auditor suggests merging two overlapping awareness modules. Training records show every in-scope person completed training. A-06 | One leaver kept access for five days past the leaving date; the other 37 leavers were removed on time.
Checks the server runs (3)
- Every line in the fixture carries a verdict

Check names are all you see here: the expectations that decide pass or fail, and the reference solution, stay on the server.
Step-by-step hints
